All patients who have a chronic health disease are eligible for a specially designed management plan, designed by your Doctor to help reduce identified health risks.

  • If you have a chronic medical condition, your Doctor may suggest a GP Management Plan.
  • If you also have complex care needs and require treatment from two or more other health care providers, your Doctor may suggest Team Care Arrangements as well.
  • Your Doctor or practice staff must obtain your agreement before providing these plans.
  • If a provider accepts the Medicare benefit as full payment for the service, there will be no out-of-pocket cost. If not, you will have to pay the difference between the fee charged and the Medicare rebate.
  • If you have both a GPMP and TCAs prepared for you by your Doctor, you may be eligible for Medicare rebates for certain allied health services. It is up to the Doctor to determine whether you are eligible for these allied health services which must be directly related to the management of your chronic condition.
  • The practice nurse can provide support and monitoring between visits to your Doctor.
  • Your Doctor will offer you a copy of your plan.
  • You and your Doctor should regularly review your plan/s.

GP management plan

This is an agreed action plan between you and your Doctor. It:

  • identifies your health care needs
  • sets out the services your Doctor provides, and
  • lists what you need to do

Team care arrangement plan

You would receive a team care arrangement plan if you need treatment from your doctor and at least 2 other health professionals. The treatment must relate to your medical condition.

This will enable you to receive Medicare rebates for specific allied health services such as physiotherapy.
