Change of Personal Details 

Please keep admin staff informed of any change to your name, address, phone number, health care card, pension or Medicare number. 

Confidentiality 

Both practices are fully computerised and are committed to maintaining the confidentiality of  personal health information. A medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. 

Patient feedback 

Patient feedback is very important to both practices, to improve services to patients.  There are feedback boxes at both medical centres at reception for this purpose.   If you have any concerns or suggestions for improvements to the facilities or services, please complete the feedback form.

Complaints 

If you are unhappy or have a complaint regarding any aspect of your care, please discuss the matter with your doctor or with practice staff. If you feel that you are unable to speak to us directly, you may also contact the Health Care Complaints Commission on 1800 043 159.

Your Rights

Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health at all times and to ensure that this information is only available to authorised people.  You are entitled to request a copy of your health record. This request must be in writing and will be dealt with in accordance with our Privacy Policy.

Coffs medical Centre and Moonee Medical Centre are bound by the National Privacy Principles and handle your personal information in accordance with the Privacy Act, 1988.

We recognise the importance of ensuring patient  confidentiality and are committed to protecting your privacy.
